The first cathedral on the site was built in 1021. Count Federico da Montefeltro sponsored the fifteenth-century rebuild, whose design is attributed to Francesco di Giorgio Martini. Construction finished in 1604, and Camillo Morigia completed the west front in 1782.

A powerful earthquake on January 12, 1789, toppled the cupola. Giuseppe Valadier reconstructed it, and the work was done by 1801. The article gives the current form as Neoclassical, a Latin cross with a central nave and two aisles under a barrel vault, topped by a coffered cupola.

The cathedral holds canvases by Federico Barocci, a Nativity by Carlo Cignani and an altarpiece by Christopher Unterberger. It is part of the Historic Centre of Urbino, a UNESCO World Heritage Site since 1998.
